New Patient Registration
If you wish to apply for registration with the practice please telephone or call in and one of our reception staff will be able to help.
Our receptionist will explain the procedure to you, provide you with the relevant paperwork, and book an appointment for you to see one of our healthcare assistants.
Before your appointment, you will be asked to fill out some forms about your current and previous health conditions, the medications you take and the details of your previous practice. Medical treatment is available from the date of registration.
You will also be required to show two pieces of identification at registration, one photo ID and one as proof of address, for example bank statement or utility bill.
Part of the registration procedure includes:
- Completion of a new patient health check form, which you can obtain from reception or print one from the link below "New Patient Health Questionnaire" .
